Student Medical Forms Medical Forms<p>​A completed Student Medical Form (FORM A1420-1 attached) is required each school year, or when a medical need is identified, or when the student transfers to another school, in order to authorize the administration of prescribed medication to students who require it during school hours, or when the medication and/or dosage requirements change.<br>Specific to asthma, every student is now permitted to carry his or her asthma medication if the student has his or her parent’s or guardian’s permission.<br>Parents/guardians and adult students are responsible for ensuring that a current, completed Student Medical Form is signed by a physician and on file at the school.<br>Some students may experience severe life-threatening allergic reactions to bee, hornet, wasp or other flying insect stings or to certain foods or food ingredients such as peanuts. Announcements for Tuesday February 20, 2018 for Tuesday February 20, 2018<p style="text-align:center;"><span lang="EN-CA" style="text-decoration:underline;"><strong>TUESDAY, FEBRUARY 20, 2018</strong></span><br>●<br>Could all grade 10 students from first Semester that produced the Banting Video Billboard announcements please come to room 182 at beginning of lunch today.<br>●<br>This is a reminder that you must pay your Student Activity Fee in order to participate in ANY school activity – this includes prom!   The cost is $15.00 and can be paid using cash online or with cash or cheque in the Main Office.<br>●<br>Members of the boys basketball team are reminded to return their washed uniforms to Mr. Wright by Thursday.<br>●<br>The beautiful game is quickly approaching.  There will be a mandatory meeting for all girls interested in trying out for the senior girls soccer team at lunch this Friday.  You must have a permission to participate form, insurance waiver form and paid student fees in order to play.  We will be in the main gym February 28th and March 7th.<br>●<br>Senior Drama Club meets today at 1100am.<br>●<br>Students involved or wishing to be involved in the Adjala Tosorontio Barn Photography Project are to meet Mrs Bell in the main office today at 11am.
