Student Medical Forms Medical Forms<p>​A completed Student Medical Form (FORM A1420-1 attached) is required each school year, or when a medical need is identified, or when the student transfers to another school, in order to authorize the administration of prescribed medication to students who require it during school hours, or when the medication and/or dosage requirements change.<br>Specific to asthma, every student is now permitted to carry his or her asthma medication if the student has his or her parent’s or guardian’s permission.<br>Parents/guardians and adult students are responsible for ensuring that a current, completed Student Medical Form is signed by a physician and on file at the school.<br>Some students may experience severe life-threatening allergic reactions to bee, hornet, wasp or other flying insect stings or to certain foods or food ingredients such as peanuts. Announcements for Monday January 15, 2018 for Monday January 15, 2018<p style="text-align:center;"><span lang="EN-CA" style="text-decoration:underline;"><strong>MONDAY, JANUARY 15, 2018</strong></span></p><p style="text-align:center;"><span lang="EN-CA" style="text-decoration:underline;"><b></b></span><br>Today's ME TO WE meeting is cancelled - we will resume meeting next semester<br>●<br>The University of Waterloo Mathematics Contests are coming to Banting on February 27.<br>Any interested grade 9, 10 or 11 students are required to sign up by providing their name, grade and a $2 deposit at the Math Office, Room 225 and pick up an information sheet.  Deadline to register is Tuesday, February 6.<br>●<br>There will be a meeting on Wednesday for the fall girls basketball team to discuss a team warm-up top. The meeting will be at 10:45 in room 240. We are hoping to finalize the design for the warm-up top. Again, there is a meeting on Wednesday at 10:45 to discuss the girls basketball team top.<br>●<br>Congratulations goes out to the Senior Girls Volleyball Team for their 3-1 regular season win this past Thursday against Georgian Bay.  <br>In addition, the Senior Girls Volleyball Team competed in a tournament at CCI this past Saturday and finished in 2nd place overall despite only 7 players participating.  The girls played 5 straight matches and it was some of the best volleyball seen yet this season.  Coaches Dasek and Siska commend Emma Spence, Jess Boyle, Alex Shannon, Clare Dasek, Cass Armstrong, and Kate Parsons for their non-stop effort and commitment to the team.  A special thank you goes out to Junior Dayna Spence for her huge contribution to a great day of volleyball.<br>Everyone is invited to come support the Junior and Senior Girls Volleyball Teams TODAY after school in the double gym, as they return to their regular season and take on Le Caron.<br>●<br>Important reminder to all students!  You must pay your student activity fee in order to participate in any school, ANY school activity.  The cost is $15.00 and can be paid using cash online or with cash or cheque in the Main Office.<br>●<br>Are you going to be 18 before June 2018?  Then you should think about applying for an exciting opportunity to gain valuable volunteer experience and be part of the reconciliation process with Canada's indigenous peoples.  Habitat for Humanity is sponsoring a unique experience this summer to go to northern Canada and help build decent affordable housing on reserve.
