Student Medical Forms Medical Forms<p>​A completed Student Medical Form (FORM A1420-1 attached) is required each school year, or when a medical need is identified, or when the student transfers to another school, in order to authorize the administration of prescribed medication to students who require it during school hours, or when the medication and/or dosage requirements change.<br>Specific to asthma, every student is now permitted to carry his or her asthma medication if the student has his or her parent’s or guardian’s permission.<br>Parents/guardians and adult students are responsible for ensuring that a current, completed Student Medical Form is signed by a physician and on file at the school.<br>Some students may experience severe life-threatening allergic reactions to bee, hornet, wasp or other flying insect stings or to certain foods or food ingredients such as peanuts. Tickets are on sale in the Pirate Pit at lunch from Monday-Thursday for $10! <br> <br>You also have the option of pre-purchasing Pizza for $2/slice, Chips for $1 a bag or a combo of both for $2.50 with your ticket. <br> <br>The out-of-school guest forms can be picked up in the Front Office. Please remember that in order to purchase a ticket you MUST have paid your student activity fee.<br><strong>●</strong><br>Do you come to school and forgot to eat something? If so, head on over to room 165 at 7:45 before classes begin to get your FREE Breakfast from Breakfast Club. That's the hospitality room 165. No reason to be hungry.<strong>●</strong><br><strong>●</strong><br>World hunger, homelessness, poverty, discrimination, climate change, food safety, bullying. The list of issues that we face today goes on and on. If you want to make a difference globally and locally join Banting's ME TO WE club. We meet today in room 148 at 11:15.
