Student Medical Forms Medical Forms<p>​A completed Student Medical Form (FORM A1420-1 attached) is required each school year, or when a medical need is identified, or when the student transfers to another school, in order to authorize the administration of prescribed medication to students who require it during school hours, or when the medication and/or dosage requirements change.<br>Specific to asthma, every student is now permitted to carry his or her asthma medication if the student has his or her parent’s or guardian’s permission.<br>Parents/guardians and adult students are responsible for ensuring that a current, completed Student Medical Form is signed by a physician and on file at the school.<br>Some students may experience severe life-threatening allergic reactions to bee, hornet, wasp or other flying insect stings or to certain foods or food ingredients such as peanuts. Thanks Banting for your generous donations!</p><p style="text-align:center;">●</p><p style="text-align:center;">Have no closet space?  Need to get rid of some clothes?  From now to Monday December 18th, a group of Gr.12 students will be collecting cloth based items for the Kidney Foundation of Canada. This will help fund the much needed research to fight against kidney based diseases. The items to consider bringing include all gently used clothing, footwear, belts, ties, purses, blankets, bedding, linens, draperies, pillows, cushions, luggage, and sleeping bags. Bring your items to your second period class.  Students will be around second period classes to collect black garbage bags and answer any questions regarding the donations.</p><p style="text-align:center;">●</p><p style="text-align:center;">Don't know what to get your friend for Christmas?  How about a Candy gram?  The Leadership class will be selling candy cane grams in the cafeteria Wednesday, Thursday and Friday this week. </p><p style="text-align:center;">Hey Banting! </p><p style="text-align:center;"> Spirit Week is here!
